LIFESPAN benefits are generous and personalized, to give our employees the coverage they need to keep them healthy and happy. From our dental program to our comprehensive health and vision care programs, LIFESPAN lets you pick and choose the options that are right for you. Whatever the case, we have you covered.
This is only a summary of your LIFESPAN benefits. Employees must refer to the Summary Plan Documents for complete information on each benefit. Certain community-based direct care positions are ineligible for LIFESPAN’s benefit package. Please check with the hiring manager as to whether the job you are applying for is a benefitted position.
Group Medical
Group medical insurance is available for all eligible full-time employees who work 30 or more hours per week. Full Time employees are eligible to participate on the first day of the month after completing 60 days of employment. Plan coverage includes: $25 per visit co-pay to primary care physician; $40 per visit specialist, with no referral required; 2-tier prescription plan ($10 Generic and $40 + 10% Name Brand)
Voluntary Dental
Voluntary dental insurance is available through Flexible Benefits for all eligible employees who work 30 or more hours per week. Employees may participate on the first day of the month after completing 60 days of employment. Plan coverage includes: Preventive care covered 100% with no deductible; Basic Care – covered at 80% with a $50 annual deductible per person; Major Care – covered at 50% after $50 annual deductible (one year wait unless employee has proof of prior coverage for one year prior to eligibility).
Group Life and STD/LTD Disability
LIFESPAN provides a $25,000 life insurance policy for all eligible full-time employees on the first of the month following 90 days of employment. On the first of the month after completing 12 months of full-time employment, employees become eligible for company paid Short-Term Disability (STD) and Long-Term Disability (LTD) benefits. Group life and Disability coverage is completely paid for by LIFESPAN.
Voluntary Vision
Voluntary vision insurance is available through Guardian-VSP for all eligible employees who work 30 or more hours per week. Employees may participate on the first day of the month after completing 90 days of employment. Coverage includes: Eye Exams – $10 copay ($46 max after $10 copay) $100 maximum allowance for lenses and frames (may substitute for contact lenses)every 12 months.
Voluntary Life Insurance
On the first of the month after completing the 90-day probationary period, employees working 30 or more hours per week may purchase additional life insurance for dependents and self.
Annual Leave - Paid Time Off (PTO) Bank
All eligible exempt and nonexempt employees who work 30 or more hours per week earn paid leave beginning the first day of employment. Leave may not be taken until completion of the 90 day probationary period. Accrual rates are based on years of service.
Retirement Investments
LIFESPAN provides employees a tax deferred 403(b) Retirement Savings Plan with matching contributions made each pay period after the first year of employment at the rate of $.50 per $1.00 of employee contributions up to 3% of gross salary. All employees are eligible to participate from the first day of employment, but do not receive matching funds until after one year of employment and 1000 hours of service. Employees are 100% vested after 3 years of employment.
Flexible Spending Account
The Flexible Spending Account (FSA) allows eligible employees to pay certain expenses with tax-free benefit dollars and is available to all employees. Employees working 30 hours or more each week are eligible to participate on the first day of the month after completing 90 days of employment. The plan features automatic reimbursement via the FSA debit card. Participation entails a bi-weekly administration fee. There is a $3,500 maximum contribution per year limit for medical reimbursement and a $5,000 limit for dependent care.
Child Care
In areas where LIFESPAN operates Circle Schools, LIFESPAN offers the opportunity for reduced cost childcare to employees.
Holiday Schedule
All eligible full-time employees are eligible to receive Holiday Pay beginning the first day of employment. The following holidays are typically observed as paid holidays for eligible employees.

- Martin Luther King's Birthday
- Memorial Day
- Three floating holiday's to be determined annually
Banking Benefits
LIFESPAN partners with two financial institutions to provide employees with reduced cost financial services.
Employee Referral Program
LIFESPAN employees may earn up to a $500 referral bonus if the employee refers a candidate to LIFESPAN for employment and the candidate is hired.
Service Award Program
LIFESPAN recognized and honors all eligible employees (part-time and full-time) who meet annual anniversary milestones. Awards change in value according to tenure with LIFESPAN.
