LIFESPAN Creative Campus programs offer a range of services to individuals with developmental and intellectual disabilities, substance use and mental health disorders. We are committed to helping each individual accomplish skills that may have seemed impossible to some utilizing person-centered plans.

Alternative Family Living Services
An option for individuals choosing to live in a natural home environment. In an AFL home, the family serves as a natural support system providing care, habilitation, and community access and supervised supports for adult(s) in the home.
In-home Skill Building
Provides habilitation and skill building support in the private home of the individual to assist the individual in becoming more independent.
Community Networking services provide individualized day activities in an integrated community setting, separate and apart from the individual’s residence and licensed facility. Services are designed develop natural supports within integrated setting to promote maximum participation in community life. Examples of Community networking include, but not limited to, participation in adult education, volunteer work in the local community, learning how to use public transportation, and participation in community groups or organizations.
This service provides support, supervision, and engaging in activities of daily living, and assists in maintaining skills learned through habilitation services.
Day Support Services
Day Supports is a service that assists the individual with acquiring and developing their self-help, socialization, and adaptive skills. Day Supports may be coordinated with any physical, occupational, or speech therapies listed in the Individual Support Plan to aid the individual with maintaining his or her maxim functional level. Services may be provided for one individual, or in a group setting.
